Key Responsibilities

  • Cook psycho-social support for patients during hospitalization
  • Develop and implement personalized discharge plans
  • Assess patient needs related to care coordination, resources, and services
  • Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction
  • Facilitate access to community resources to support recovery and continuity of care
  • Ensure compliance with hospital policies, regulatory standards, and evidence-based practices

Qualifications

  • Education: Bachelor’s degree in Social Work required, Master’s degree preferred
  • Experience: Minimum two years in acute care, SNF, physician offices, or community health settings
  • Licensure/Certification: LBSW license in Maryland required, must obtain care/case management certification within five years of hire

Skills & Abilities

  • Strong communication and multitasking skills
  • Familiarity with EMRs and ability to navigate electronic medical records
  • Knowledge of disease processes, regulatory requirements, and evidence-based care
  • Understanding of community resources, provider networks, and the revenue cycle
  • Ability to build relationships across clinical and community settings
