Key Responsibilities
- Cook psycho-social support for patients during hospitalization
- Develop and implement personalized discharge plans
- Assess patient needs related to care coordination, resources, and services
- Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction
- Facilitate access to community resources to support recovery and continuity of care
- Ensure compliance with hospital policies, regulatory standards, and evidence-based practices
Qualifications
- Education: Bachelor’s degree in Social Work required, Master’s degree preferred
- Experience: Minimum two years in acute care, SNF, physician offices, or community health settings
- Licensure/Certification: LBSW license in Maryland required, must obtain care/case management certification within five years of hire
Skills & Abilities
- Strong communication and multitasking skills
- Familiarity with EMRs and ability to navigate electronic medical records
- Knowledge of disease processes, regulatory requirements, and evidence-based care
- Understanding of community resources, provider networks, and the revenue cycle
- Ability to build relationships across clinical and community settings
