JOB SUMMARY
The Social Worker (BSW) supports the intake and admissions process for a Certified Home Health Agency (CHHA) by assisting patients, families, referral sources, and internal staff with psychosocial coordination, resource navigation, and care support services. This position works closely with the intake team to help ensure timely, efficient, and compassionate onboarding of patients into home care services. This is a strictly office-based role and does not require field visits or a social work license.
RESPONSIBILITIES
- Collaborate closely with the intake team to support new patient referrals and admissions.
- Conduct initial psychosocial screenings and gather relevant social history information from patients, families, hospitals, and referral sources.
- Aid patients and caregivers in identifying community resources and support services related to home care needs.
- Communicate with hospitals, physician offices, discharge planners, and community agencies to coordinate services and obtain required documentation.
- Provide emotional support and guidance to patients and families during the intake and transition-to-home-care process.
- Affirmatively assist with insurance verification support and collection of necessary intake.
- Maintain accurate and timely electronic records, intake notes, and communication logs.
- Ensure all documentation complies with agency policies, HIPAA regulations, and applicable state/federal requirements.
- Participate in intake meetings, case discussions, and interdisciplinary communication as appropriate.
- Respond professionally and compassionately to incoming calls and inquiries from patients, families, and referral sources.
- Support administrative and operational functions of the intake department as assigned.
QUALIFICATIONS
- Bachelor’s Degree in Social Work (BSW) required.
- Social work license is not required for this position.
- Minimum one (1) year of experience in healthcare, home care, case management, intake coordination, customer service, or social services preferred.
- Experience working in a CHHA, home health, hospital discharge planning, or healthcare office setting preferred.
- Strong interpersonal, communication, and organizational skills.
- Ability to manage multiple priorities in a fast-paced office environment.
- Proficiency with electronic medical records (EMR) systems and Microsoft Office applications.
- Knowledge of community resources, healthcare terminology, and patient service coordination preferred.
- Ability to maintain confidentiality and professionalism at all times.
