Social Worker-BSW
About the Role
This position develops, coordinates, and provides social work services to patients and families facing social, emotional, and situational stressors due to illness, injury, and/or disability. The goal is to aid in adaptation and empower patients and families to participate fully in care coordination and/or discharge planning. The role provides developmentally appropriate care, including planning for safe discharge and continuity of care, while addressing the unique needs of all ages, as well as physically disabled, mentally ill, chronically ill, and terminally ill patients.
Responsibilities
- Processes, plans, and facilitates the coordination of care and/or timely discharge/transfer of patients from acute/observation care.
- Assesses and evaluates patient needs within the framework of their functional abilities, culture, and available support/resources.
- Facilitates and implements the care plan in collaboration with the healthcare team.
- Collaborates with all members of the healthcare team to develop, manage, and communicate the transition of care plan.
- Documents all interventions in the patient medical record accurately and timely, including all elements of the transition of care plan.
- Assists and supports patients and families, providing advocacy and resource services.
- Acts as a liaison to healthcare facilities and community groups.
- Maintains knowledge of Medicare, Medicaid, and other program benefits to assist patients with discharge planning and choices.
- Applies knowledge of behavioral health theories, end-of-life dynamics, grief and bereavement counseling, substance abuse identification, and interventions.
- Supports victims of abuse, neglect, or violence.
- Provides professional education to staff and the community.
- Performs all functions according to established policies, procedures, regulatory, and accreditation requirements, as well as professional standards.
- Demonstrates core and leader behaviors to provide an excellent service experience for all customers of Foundation Health.
Requirements
- Bachelor’s Degree in Social Work.
- Non-temporary Alaska LBSW license required within 12 months of hire; temporary LBSW license acceptable within 90 days of start/transfer.
- Knowledge of government/community agencies and resources, such as Medicare/Medicaid, long-term care, or other applicable services.
- Effective communication, human relation, and time management skills.
- Ability to work flexible hours and take rotation call after hours/weekends in acute care settings.
Preferred Qualifications
- BLS Certification.
- Additional related education and/or experience.
Pay
$25.88 to $40.44 hourly wage based on experience and education.
- Shift differentials: $2.00/hr for weekends, $2.50/hr for evenings, $6.00/hr for nights.
- Annual increases and paid time off.
- Retention bonus available.
- Relocation assistance up to $7,500.
Schedule
- Full-time, 40 hours per week with weekend rotation.
- 5X8-hour shifts with required call coverage.
- 90 days of temporary housing provided.
