About the role
Bienvivir All-Inclusive Senior Health is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. The Bachelor Social Worker (BSW) is responsible for providing outreach, social services, advocacy, and case management to participants enrolled in the program and their families.
Responsibilities
- Provides case management services to Bienvivir participants, including home safety screens, liaison with families, and referrals to community resources.
- Assists Master Social Worker (MSW) as needed to complete placement cost contracts and submit for approval.
- Completes medical alert referrals, disenrollment documents, and MSUR Service Delivery Day Unit forms as needed.
- Attends daily IDT meetings, care plan meetings, Social Work Department meetings, and CEO bi-weekly meetings via ZOOM.
- Provides ongoing communication to participants/decision-makers regarding Participant Rights, Grievance and Appeals Process, and Care Planning.
- Supports MSW with completing Mini-Mental State Exam (MMSE) as scheduled and alerts MSW to concerns related to Behavioral Health.
- Establishes and maintains positive relationships with contracted facilities and agencies to ensure timely placement or respite for participants.
- Works with the Intake Team to initiate and process financial and medical applications for potential participants.
- Provides education to visitors and their families regarding Advanced Directives, Medical Power of Attorney, and Guardianship.
- Collaborates with Enrollment Team Physician and Nurse to refer visitors to placement options as appropriate.
- Familiarizes self with, understands, and implements Social Services department policies and procedures.
- Provides mentorship to Social Work Interns on Bienvivir’s Mission and core values.
Qualifications
- Graduate of an accredited university with a bachelor’s degree in social work or related field.
- Preferred: licensed by the Texas State Board of Social Worker Examiners within six months of employment.
- One year of experience providing Social Services to a frail or elderly population preferred.
Skills
- Knowledge and experience working with the geriatric population and family systems.
- Knowledge of community referral system for community services.
- Knowledge and experience working with case management.
- Ability to communicate effectively to participants and community groups the philosophy, objective, and policies of Bienvivir Senior Health Services.
- Works well within the ethical principles of the Social Work Code of Ethics.
- Ability to work as a member of an interdisciplinary team and communicate effectively with staff, participants, and family members.
- Bilingual (Spanish/English) required.
- Dependable transportation, valid driver’s license, and automobile liability insurance required.
- Knowledge of Microsoft Office, Zoom, and Teams applications.
