Social Worker - Behavioral Health Patient Navigator - ER - Days

Essential Job Duties

• Affords patients clarity on their treatment options and resources available within the Baptist system and the broader community.
• Facilitates resolution of barriers to accessing primary psychiatric and preventive healthcare services before discharge.
• Maintains positive relationships with the behavioral healthcare community and other businesses in the community to ensure patients receive necessary care and services.
• Trains patients on accessing behavioral healthcare services, including roles of various outpatient resources after discharge, to support optimal recovery outcomes.
• Facilitates discharge planning and connects uninsured/underfunded patients with financial counselors and enrollment services when necessary.
• Keeps accurate records of patient contacts, referrals, and services provided, and is knowledgeable about the policies governing the use of this information in the Electronic Health Record.

Required Education And Experience

• Bachelor’s Degree in Social Work
• 1-3 years of related experience

Required License/Certifications

• Licensed Social Worker in State of Texas
