About the role
We are seeking a dedicated and compassionate Social Worker to join our team at Pawtucket Falls Health Care in Pawtucket, Rhode Island.
Responsibilities
- Assess the social and emotional needs of residents, including their medical, psychological, and emotional well-being
- Develop and implement individualized care plans to address the unique needs of each resident
- Provide counseling and support to residents, families, and caregivers to help them cope with challenging situations
- Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to develop and implement comprehensive care plans
- Identify and address potential social and emotional issues, such as depression, anxiety, and isolation
- Maintain accurate and up-to-date records of resident care and services
Requirements
- A bachelor's degree in Social Work or a related field
- A minimum of [minimum years of experience?] years of experience in social work or a related field
- Licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) in the state of Rhode Island
- Strong communication, interpersonal, and problem-solving skills
