Primary Responsibilities
- Aids in understanding significant personal, emotional, environmental and social factors related to the patient's health status.
- Works with the health care team to develop a comprehensive, integrated Plan of Care for patients.
- Instructs health care team members on community resources available to assist patients.
- Assumes responsibility as Bereavement Coordinator and supervises the provision of bereavement services.
- Develops a Plan of care addressing bereavement needs with clear delineation of services.
Required Qualifications
- Masters Degree in Social Work from an accredited institution.
- Current CPR certification.
- Licensed Social Worker in the state of residence.
- Valid Driver’s License, vehicle insurance, and access to a reliable vehicle or public transportation.
- One year of social work experience in a healthcare setting.
Preferred Qualifications
- Bereavement Coordination experience.
- Experience with establishing a plan of care for bereavement needs.
Pay and Benefits
Pay is based on several factors including local labor markets, education, work experience, certifications, etc. The salary for this role will range from $76,360 to $114,540 annually based on full-time employment. Additional benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility).
