Social Worker
About the role
The Social Worker is assigned to the HUD-VASH section of mental health at the Montrose VAMC. The HUD-VASH is an intensive case management program for chronically homeless Veterans aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system. The HUD-VASH Social Worker is responsible for providing case management for a caseload of Veterans in the HUD-VASH program. Provides psychosocial screening, assessment, counseling and crisis intervention. Develops a comprehensive treatment plan in collaboration with the Veteran. Coordinates with family conferences and provides education to families and Veterans. Responsible for reviewing agreements between Housing Choice Voucher (HCV) landlords, Public Housing Authorities and the VA; ensures Veteran is eligible for program participation; and performs site visits of HUD-VASH apartments.

Requirements
The Social Worker must be proficient in written and spoken English.
Qualifications
- Must be proficient in written and spoken English.
- Must be licensed or certified by a state to independently practice social work.
- Must have a masters degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
- Must have a doctoral degree in social work from a school of social work to substitute for the required one year of professional social work experience in a clinical setting.
