About the Role

The primary purpose of this position is to plan, organize, and implement social services for PACE Organization (PO) participants and families. Responsibilities include assessment, treatment, teaching, and counseling participants, caregivers, or other appropriate representatives. Social Worker interventions may involve individual participant contacts, collateral contact, participant and family education, assessment and counseling, provision of resources, addressing mental health needs, ongoing case management, advocacy, and disenrollment procedures.

Responsibilities

• Conduct comprehensive assessments and develop tailored treatment plans.
• Provide counseling, education, and support to participants and their families.
• Collaborate with multidisciplinary teams to address diverse needs effectively.
• Advocate tirelessly to ensure participant and caregiver needs are met.
• Navigate disenrollment procedures with empathy and professionalism.

Requirements

• Master's degree in social work with two years of relevant experience.
• State social work licensure preferred.
• Experience in geriatric social work is highly valued.
• Proficiency in social service principles and funding mechanisms.
• Valid driver's license and reliable transportation.
• Ability to navigate physical demands with ease.
