About The Role

The Social Worker, holding an MSW and LCSW, plays a critical role in providing comprehensive support and advocacy for individuals and families facing various social, emotional, and psychological challenges. This position is designed to deliver client-centered interventions that promote well-being, resilience, and access to necessary resources within the community. The Social Worker will collaborate with multidisciplinary teams to develop and implement individualized care plans that address clients' unique needs and goals. Through direct counseling, crisis intervention, and case management, the role aims to empower clients to overcome barriers and improve their quality of life. Ultimately, this position contributes to fostering a supportive environment that enhances social functioning and promotes positive outcomes for diverse populations served.

Responsibilities

  • Conduct comprehensive psychosocial assessments to identify client needs, strengths, and challenges.
  • Develop, implement, and monitor individualized care plans in collaboration with clients and interdisciplinary teams.
  • Provide counseling and therapeutic interventions to individuals, families, and groups to address mental health, behavioral, and social issues.
  • Advocate for clients by connecting them with community resources, healthcare services, and support networks.
  • Maintain accurate and timely documentation of client interactions, progress notes, and case management activities.
  • Participate in multidisciplinary team meetings to coordinate care and ensure holistic support for clients.
  • Conduct crisis intervention and provide support during emergencies to ensure client safety and stability.

Qualifications

  • Master of Social Work (MSW) degree from an accredited institution.
  • Licensed Clinical Social Worker (LCSW) credential valid in the United States.
  • Minimum of two years of clinical social work experience providing direct client services.
  • Strong knowledge of social work principles, ethics, and relevant federal and state regulations.
  • Proficiency in conducting assessments, developing care plans, and delivering evidence-based interventions.

Preferred Qualifications

  • Experience working with diverse populations including children, adults, and elderly clients.
  • Familiarity with electronic health record (EHR) systems and case management software.
  • Training or certification in trauma-informed care, crisis intervention, or substance abuse counseling.
  • Bilingual abilities or experience working with non-English speaking clients.
  • Experience collaborating within multidisciplinary healthcare or social service teams.

Skills

  • Strong communication and interpersonal skills are essential for building trust with clients and collaborating with healthcare professionals and community partners.
  • Organizational and documentation skills ensure accurate record-keeping and compliance with legal and ethical standards.
  • Preferred skills such as bilingualism and trauma-informed care enhance the ability to serve diverse populations with sensitivity and cultural competence.
  • Proficiency with technology, including EHR systems, facilitates efficient case management and coordination of services in a fast-paced environment.
