About the role

• Children with Differing Abilities (CWDA) – Children's Long-Term Support-Waiver (CLTS) Social Worker: Conduct needs assessment, case planning, and advocacy; provide and coordinate services to children with varying disabilities, functioning levels, and mental health needs.
• Comprehensive Community Services (CCS) Social Worker – Children: Perform needs assessment, crisis intervention, case planning, advocacy, and development of community resources; serve children and adults with varying disabilities, functioning levels, substance use, and mental health needs using a wrap-around/multi-disciplinary team approach.
• Adult Comprehensive Community Services (CCS) Social Worker – Adults: Conduct needs assessment, crisis intervention, case planning, advocacy, and development of community resources; serve children and adults with varying disabilities, functioning levels, substance use, and mental health needs using a wrap-around/multi-disciplinary team approach.

Requirements

• Minimum of one (1) year of full-time or part-time work experience
• Valid driver’s license
