Responsibilities
- Reports any changes in licensure to their direct supervisor.
- Identifies members at risk for psychosocial needs related to health care status.
- Completes contacts via telephone calls to follow-up on referrals, assess psychosocial needs, and provide interventions.
- Affords community resource referrals to The Health Plan members and/or their supportive others.
- Maintains accurate and timely documentation of contacts, needs assessments, interventions, and outcomes in Case Tracker.
- Collaborates with Health Plan staff in providing practitioner and facility education about available support services.
- Develops and implements programming for social intervention consistent with identified needs of specific member populations.
- Maintains competency in chronic disease navigation and health maintenance issues.
- Promotes communication within and outside The Health Plan to enhance medical management services.
- Identifies opportunities for improvement in systems, processes, functions, programs, and procedures.
- Prioritizes assignments and maintains flexibility as new priorities arise.
- Participates in Grand Rounds, Transitional Care, and Post Discharge Outreach.
Qualifications
- Required: Social Work or related undergraduate degree with active licensure as a social worker in Ohio or West Virginia upon hire.
- Desired: Master of Social Work, Case Management Certification (CCM), or Certified Advanced Social Work Case Manager.
