Social Worker

Requirements

• MSW from an accredited Social Work school
• Active/valid NJ license at submission
• Hospital experience required
• Inpatient adult discharge planning experience required
• Psychosocial assessment and support
• Social determinants of health
• Multidisciplinary rounds
• Advanced care planning

Responsibilities

• Assess patients' psychosocial needs
• Provide support to patients and families
• Participate in multidisciplinary rounds
• Coordinate discharge planning and after-care services
• Connect patients with community, financial, housing, hospice, rehab, SNF, and other resources
• Document assessments and discharge planning
