About the role
Shriners Children’s St. Louis is seeking candidates for a PRN Social Worker to join our Care Management team. The Social Worker is responsible for providing assessments, planning, interventions, and follow-up for patients and family members across the continuum of care. Through this role, you'll have the opportunity to make an impact on our patients' experience by providing continuity of care for positive outcomes in our family-centered environment.
Responsibilities
- Documents assessments, interventions, plans, and evaluations in accordance with regulatory agency requirements and SHC policy.
- Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.
- Advocates by supporting or recommending services for the protection of the patient's health, safety, and rights.
- Collaborates by working with patients, families, caregivers, and the health care team locally and within the system to jointly communicate, problem-solve, and share accountability for optimal outcomes.
- Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.
Requirements
- Master’s degree in Social Work
- Social Work License in State of Practice
Qualifications
- Case Management Certification (CM, ACM, or ANCC) preferred
- 3 years of Social Work experience preferred
Pay
Compensation is determined based on years of relevant experience and departmental equity.
