Social Worker

About the role

Sapphire Center for Rehabilitation and Nursing, a premier skilled nursing facility located in Queens County, is seeking a full‑time Social Worker (MSW). As a Social Worker for Sapphire Rehab, you will support the facility in our mission of creating and maintaining an environment that promotes quality care of residents’ physical, emotional, and social well‑being.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct social service programs in accordance with federal, state, and local standards and facility policies to meet residents’ medically related emotional and social needs.
• Perform resident evaluations as required by state regulations.
• Support state regulations for admission and retention of residents.
• Assist residents and families with the move‑in process.
• Orient residents and families to the facility.
• Assist and interact with residents and families to access services.
• Conduct discharge planning and assistance with community living/homecare setup.
• Participate in development and implementation of each resident's individual service plan.
• Communicate with other members of the management team to ensure coordination of services to residents.
• Document case management notes in residents' files as necessary.
• Oversee and coordinate the Individualized Service Plan (ISP) and the ALR personal data and resident evaluation form.
• Perform other duties as assigned.
• Maintain knowledge of MDS and discharge plans.

Requirements

• Knowledge of age‑specific developmental factors for adult and geriatric residents (physical, cognitive, and socialization) to direct the facility’s service delivery.
• Professional, compassionate, and outgoing individual with work experience in the social services field and experience in skilled nursing/long‑term care setting.

Qualifications

• MSW or Master’s degree in a related field required.
• Licensed and credentialed as a social worker by the New York Department of Education.
• Experience in human services or social work providing services to an adult population as required by state regulation (if applicable).
• Basic computer knowledge, particularly in Microsoft Word.
