About the role
Regal Heights Nursing Center is looking for an experienced Social Worker to join our caring team!
Qualifications
- Nursing Home or Hospital experience (internship is sufficient) — Required
- BSW or MSW in related field — Required
- Bi-lingual English/Spanish — Preferred but not required
Responsibilities
- Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needs
- Initiating appropriate interventions for residents experiencing mental or behavioral health issues
- Identifying individual social and emotional needs
- Participate in the planning of the resident's admission, community re-entry, or transfer to another facility
- Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay
- Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community
