Job Duties
- Affords a variety of social work services within commonly accepted standards of social work practice, ranging from simple to crisis cases.
- Affords assessments of the needs of patients and their families and develops appropriate programs/services.
- Investigates and determines the kinds of services needed by an individual client and/or family; carries out approved treatment plans; prepares case records and reports.
- Uses effective interpersonal skills in managing complex interactions involved in the functioning of a multidisciplinary team.
- Serves as a resource to clinical staff in managing complex psychosocial issues and accessing community resources.
- Creates resources for the integrated health care team based on the needs of their patients.
- Applies the principles, practices, and codes of ethics of social work services in the performance of assignments such as assessing, developing, and providing direct services or programs to individuals, families, and/or groups.
- Affords assistance with medication, transportation, housing, and other social determinants of health.
- Functions as an advocate for patient rights and organizational ethics.
- Possesses a strong commitment to diversity, equity, and inclusion; community engagement; and social justice.
- Identifies and assesses available services to meet the social, financial, and emotional needs that affect a patient's medical condition, treatment, recovery, and transition to a lower level of care.
- Maintains an ongoing current file of community resources and programs.
- Affords assistance with formal and informal referrals, phone calls, and completion of paperwork and applications.
- Acts as a liaison between OneWorld and community resources.
- Conducts internal and external presentations as needed.
- Provides orientation, training, and supervision to social/human services practicum students.
