About the role
We are seeking a full time (40 hours) compassionate and detail oriented Social Worker to join the team at New London Rehab and Care Center of Waterford.
Responsibilities
- Support residents and their families by addressing psychosocial needs
- Facilitate care planning
- Ensure a smooth transition throughout the continuum of care
Requirements
- Bachelor's or Master's in Social Work required
