Social Worker
About the Role
This position is remote/hybrid in the Tampa Bay/Sarasota area. The Social Worker is primarily responsible for assessing the psycho/social needs of patients, families, and caregivers, educating them and Millennium employees about available community resources, and coordinating with those resources to assist the patient. Resources include but are not limited to food, shelter, utilities, and pharmaceutical financial needs.
The role requires strong interpersonal and communication skills, as well as the ability to utilize both internal and external resources to accomplish goals and objectives.
Responsibilities
- Determine the nature of a patient’s situation by interviewing, assessing medical, psychological, emotional, and social information to assist with patient needs.
- Research and gather information regarding community resources, including pharmaceutical resources for patients needing assistance with copayments.
- Assist with patient placement to Skilled Nursing Facilities.
- Support Millennium’s employees in coordinating community resources to effectively support the patient’s care plan.
- Meet with community resources within the designated area to build relationships and assist patients in obtaining those resources.
- Maintain client confidence and protect operations by keeping information confidential.
- Travel required for in-home assessments of Social Determinants of Health and to assist patients with completing applications.

Requirements
- Bachelor’s degree in Social Work from a four-year college or university; preference for a master’s in social work.
- Ability to read, analyze, and interpret complex documents and respond effectively to sensitive inquiries or complaints.
- Ability to apply basic math as necessary.
- Strong reasoning ability to define problems, collect data, establish facts, and draw valid conclusions.
- Knowledge of Athena Database software, Microsoft Spreadsheet software (Excel), and Microsoft Word Processing software.
Physical Demands
- Regularly required to stand, walk, use hands to finger/handle, reach with hands and arms, climb or balance, and talk or hear.
- Frequently required to sit.
- Occasionally required to stoop, kneel, crouch, or crawl.
- Occasionally lift and/or move up to 10 pounds.
- Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
Work Environment
- Occasionally exposed to outside weather conditions.
- Noise level in the work environment is usually moderate, depending on the business activity of the office.
Pay
The anticipated base salary range for this position is $58,165.00 to $87,247.00. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
