You are interested in making a profound impact on the lives of underserved individuals and families living with complex and/or chronic medical and behavioral conditions. You enjoy benefits effective on the first day of employment, assisting patients in understanding their illness and coordinating their care, learning about community services and resources to promote healthy behaviors through conducting the key functions listed in the duties and responsibilities, and participating in a loan repayment program and funding for license renewals.
Responsibilities
- Address the social service needs of One Care members.
- Provide follow-up sessions as needed with One Care health center clients or coordinate services by phone.
- Provide brief counseling, advocacy, and resource referrals as needed.
- Participate in case conferences as needed.
- Conduct home visits alone or in conjunction with members of the interdisciplinary team as appropriate, either at the patient's home or in other locations in the community.
- Be an active and integral member of the primary care/BH team.
- Advocate for patients with outside agencies such as social service organizations, schools, etc.
- Provide crisis intervention as needed.
Requirements
- Human services experience desired.
- Ability to work with children, adults, and the elderly.
- Minimum of a bachelor’s degree in the human service field (e.g., psychology, sociology) required, BSW preferred; bilingual/bicultural candidates strongly desired.
- LSW required.
- History of good attendance and positive work attitude.
