Requirements

• Degree in social work
• Minimum of one year experience in long-term care, skilled nursing, mental health, healthcare, or human services
• Strong communication, documentation, and resident support skills
• Ability to work collaboratively with nurses, psychologists, psychiatrists, and healthcare professionals

Responsibilities

• Provide emotional, psychological, rehabilitation, and resident support services under the direction of nursing and medical staff
• Maintain accurate documentation of patient condition, behavior, eating habits, and daily activities
• Collaborate with interdisciplinary healthcare teams including nurses, psychologists, and psychiatrists
• Interview residents upon admission and complete assessments and documentation
• Participate in recreational and social activities with residents to encourage engagement and quality of life
