Responsibilities

• Provide assessment, planning, consulting, education and referral services to Gundersen Health System residents and family members.
• Act as consultants and leaders in comprehensive care throughout the system, while supporting organizational strategies and goals.

Requirements

• Bachelor’s degree in Social Work or in a related field.
• MN Social Worker license is required before starting employment.
