About the role

GGRC is committed to assisting people with intellectual and developmental disabilities lead lives of liberty and opportunity. This role offers a hybrid schedule, allowing you to work with clients in their homes in San Mateo County, from our San Mateo office, and from your own home.

Responsibilities

  • Using Person Centered practices, assess the needs of developmentally disabled individuals and their families and develop and implement the person's Individual Program Plan (IPP) and coordinate associated services.
  • Manage a wide range of active cases requiring professional social work skills, including creative planning to seek and acquire services for individuals supported.
  • Meet with individuals at their residence or in the community to design, develop, implement, monitor, and revise the IPP.
  • Assess needs and identify barriers to individuals' developmental objectives.
  • Initiate and terminate regional center funding and ensure purchased services are provided.
  • Prepare required reports and documentation in a timely and accurate manner, maintaining records in compliance with federal and state requirements.
  • Act as an advocate for individuals supported and their families to ensure receipt of entitled services and protection of client rights.
  • Encourage individuals and their families to participate in coordinating and utilizing available resources.
  • Review and ensure individuals receive appropriate and effective services from existing community resources.
  • Initiate referrals and monitor progress of vendored services, evaluating availability and effectiveness.
  • Identify community service needs or gaps and assist in developing needed services and improving service quality.
  • May provide intake and outreach services as assigned.
  • Participate in internal and community committees and task forces as assigned.

Requirements

  • Bachelor's Degree from an accredited school (social welfare or related human services field preferred but not required).
  • Demonstrated interest in working with persons with developmental special needs.
  • Access to reliable transportation.

Qualifications

  • Knowledge of principles, objectives, and techniques of counseling individuals.
  • Knowledge of normal and abnormal individual and group behavior, human growth and development, and family relationships.
  • Knowledge of community structure and organization and how to intervene constructively on behalf of individuals supported.
  • Knowledge in identifying and navigating generic services, counseling techniques, and typical/atypical child development behavior.
  • Ability to interview skillfully and develop independent work skills.
  • Ability to exercise judgment in decision-making and cooperate as part of an interdisciplinary team.
  • Ability to stimulate cooperation from individuals supported, their families, and community agencies.
  • Ability to speak and write effectively.
  • Ability to maintain clear and concise case records.
  • Ability to establish and maintain effective working relationships.
