Social Worker

Responsibilities

• Enrich lives as a nursing center Social Services Specialist
• Work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and obtain services to meet those needs
• Provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life
• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being
• Educate staff on the role of social services
• Develop a written, interdisciplinary plan of care for each patient/resident to address social service needs
• Hold meetings with patients/residents, families, and significant others as requested and provide support and information to improve quality of life
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population

Qualifications

• Bachelor's degree in social work or human services field required
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Posted Salary Range USD $20.00 - USD $22.00 /Hr.
