Social Worker

About the role

As a Social Worker at DaVita, you will be a vital member of each patient’s core care team, supporting and advocating for patients navigating the transition to dialysis. You will help patients understand their rights and responsibilities and guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.

Responsibilities

• Build meaningful, long‑term relationships with patients and their families in an intimate outpatient setting.
• Support patients through the challenges of dialysis, including education on rights, responsibilities, and self‑management.
• Collaborate with the care team to set clinical goals and participate in quality‑improvement initiatives.
• Provide emotional, mental, and financial counseling to patients and families.
• Maintain professional documentation and adhere to state regulations for dialysis social work.

Requirements

• Master’s Degree in Social Work (MSW) with a specialization in clinical practice from an accredited school of Social Work.
• Meets all state‑required regulations to practice in the dialysis setting.
• Obtained a clinical license (e.g., LCSW, LICSW) and meets all licensure requirements for the state.
• Proficient in experienced‑level Social Worker duties.
• Intermediate computer skills.
• Demonstrated knowledge of government and private insurance programs.
• Excellent interpersonal and communication skills.
