Social Worker

About the role

This is a part-time Social Worker position offering 24 to 30 hours/week, based on clinical needs and patient visit schedules across both DaVita Brentwood and Century City facilities. The role involves supporting patients transitioning to dialysis and helping them manage the physical, mental, emotional, and financial aspects of their condition.

Responsibilities

• Support and advocate for patients in understanding their rights and responsibilities.
• Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
• Build meaningful and long-term relationships with patients and their families in an outpatient setting.
• Be a part of a supportive and collaborative team that values work-life balance and personal and professional growth.

Requirements

• Master's degree in Social Work (MSW) with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook, with functional proficiency in DaVita specific applications.

• Meets all state required regulations to practice in the dialysis setting.

Skills

• Strong interpersonal and communication skills.
• Ability to handle sensitive and confidential patient information.
• Knowledge of end-stage renal disease management.
• Proficiency in Microsoft Office Suite.

Pay
