Responsibilities

• Help patients understand their rights and responsibilities
• Guide patients in managing the physical, mental, emotional and financial demands of End Stage Renal Disease
• Cover 2 clinics must be able to split time between them

Requirements

• Masters degree in Social Work (MSW) required with a specialization in clinical practice
• Must have LMSW or RCSW
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
• Cover 2 clinics must be able to split time between them

Pay

The Salary Range for the role is $80,000 - $125,000/year. Social Worker Intermediate: $86,000 - $93,000/year.
