Social Worker

About the role

You will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease. This can include crisis counseling, mental health services, and connecting patients with community programs and resources.

Responsibilities

• Help patients understand their rights and responsibilities related to End Stage Renal Disease.
• Counsel patients on crisis situations and provide mental health services.
• Connect patients with community programs and resources.

Requirements

• Master's degree in Social Work (MSW) required.
• Must be licensed or registered in the state of Indiana as required by state regulations - LMSW or LCSWD.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days.
• Dedication to caring for patients suffering from chronic kidney failure.

Qualifications

Pay

Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience. DaVita offers a competitive total rewards package, which includes a 401k match, healthcare coverage and a broad range of other benefits.
