About the role

As a Social Worker at DaVita, you’ll be a vital member of each patient’s core care team. You will support and advocate for patients, helping them understand their rights and responsibilities, and guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. This role allows you to build meaningful, long-term relationships with patients and their families in an intimate outpatient setting.

Responsibilities

• Support and advocate for patients navigating dialysis treatment.
• Help patients understand their rights and responsibilities.
• Guide patients in managing the physical, mental, emotional, and financial challenges of End Stage Renal Disease.
• Build long-term relationships with patients and their families.

Requirements

• Meets all state required regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
• Functional proficiency with DaVita-specific applications within 60 days.
