Location: 9200 Bear Lake Rd, Forest City, Florida, 32703-1913, United States. This full-time opportunity covers two outpatient dialysis clinics in Apopka and Ocoee. Schedule: Monday – Friday.
About the role
As a Social Worker at DaVita, you’ll be a vital member of each patient’s core care team. You will support and advocate for patients, helping them understand their rights and responsibilities while guiding them through the physical, mental, emotional, and financial demands of End Stage Renal Disease. This role allows you to build meaningful, long-term relationships with patients and their families in an intimate outpatient setting.
Responsibilities
- Support patients in managing the transition to life on dialysis.
- Advocate for patients and help them navigate their rights and responsibilities.
- Guide patients in handling the physical, mental, emotional, and financial challenges of End Stage Renal Disease.
- Work as part of a collaborative team in a positive environment.
Requirements
- Meets all state regulations required to practice in the dialysis setting.
- Master’s degree in Social Work (MSW) with a specialization in clinical practice.
- Demonstrated knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
- Functional proficiency with DaVita-specific applications within 60 days.
