Responsibilities

• Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member’s benefit plan and/or health needs.
• Research available options and resources for members with financial, transportation, safety, and housing needs.
• Empower members to make positive lifestyle and medical choices.
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
• Conduct all member contact telephonically or via electronic exchange.

Qualifications

• Must reside in North Carolina.
• Master's Degree in Social Work.
• Active, unrestricted licensure as one of the following: LSW, LMSW, LCSW, LISW.
• 2+ years of experience in medical social work or case management.
• Ability to obtain and maintain multi-state licensure (minimum of 10 states – willing to obtain additional licenses) before completing new hire training (licensure costs paid by the company).

Schedule

• Remote work‑from‑home role anywhere in the U.S.; must be able to work EST hours.
• Working hours: Monday‑Friday, 8:00 am – 4:30 pm EST.
• No weekends, holidays, or travel required.
• Anticipated weekly hours: 40.
• Time type: Full‑time.

Pay

The typical pay range for this role is $43,888.00 – $91,052.00 (base hourly rate or base annual full‑time salary). The actual base salary offer will depend on factors such as experience, education, geography, and other relevant considerations. This position is eligible for a CVS Health bonus, commission, or short‑term incentive program in addition to the base pay range.
