Social Worker

About the role

A dedicated Master Social Worker (MSW) is needed to join Continuum Hospice. The primary responsibility is to deliver exceptional social work services to hospice patients and their families, focusing on emotional support and coping capacity.

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Provide individualized care advocacy and implement personalized social work plans.
• Collaborate with the Bereavement Coordinator and attend interdisciplinary meetings.
• Address psychosocial stresses and shape care plans.
• Provide empathetic counseling to patients and families.

Qualifications

• Education: Master of Social Work from an accredited school of social work.
• Licenses: Licensed as a Social Worker from the State Board of Social Work.
• Experience: Minimum one (1) year experience as a social worker in a hospice setting and a minimum one (1) year experience as a social worker in a healthcare setting.
• Additional Requirements: Minimum one (1) year experience in long-term care or medical surgical/acute care settings preferred.
• State-Specific Requirements: Possess and maintain current CPR certification.

Pay

$51.00 - $55.00 per hour (Average Pay Range). Final compensation rate will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position.
