About the role

As our Social Worker, you'll play a vital role in helping patients and families navigate complex healthcare needs with compassion and expertise. By addressing social, emotional, financial, and discharge planning needs, you'll help remove barriers to care and support positive patient outcomes throughout the care journey. You'll collaborate with patients, families, physicians, nurses, and the interdisciplinary care team to develop individualized plans of care and coordinate safe, timely transitions. You'll serve as a trusted advocate, connecting patients with community resources and support services that promote continued health and well-being beyond the hospital. CHI Health Immanuel, located in North Omaha just off of I-680 on 72nd Street, offers a full-service hospital, cancer center, rehabilitation center, specialty spine hospital and more. Responsibilities

• Assess patients' psychosocial, environmental, financial, and discharge planning needs to develop individualized plans of care.
• Collaborate with the interdisciplinary care team to coordinate safe, effective discharge plans and facilitate timely referrals to appropriate post-acute and community resources.
• Advocate for patients and families by connecting them with support services and assisting with applications for available community and financial resources.
• Educate patients, families, and healthcare team members on available services, care options, and community resources to support successful transitions of care.
• Document assessments, interventions, and care coordination activities while maintaining compliance with organizational, state, and federal standards.

Requirements

• Certified Social Worker (CSW) in Nebraska
• Minimum of a bachelor's degree in Social Work (BSW), with the intention of entering into a master's degree program in Social Work (MSW)
• Basic Life Support (BLS) from the American Heart Association
