Social Worker

Responsibilities

• Conduct psychosocial assessments for patients and families
• Develop discharge and care plans
• Coordinate community resources and provide crisis intervention and counseling
• Collaborate with interdisciplinary teams to address social determinants of health
• Advocate for patient needs and ensure safe transitions of care
