Social Worker

About the role

The NJ Department of Human Services, Division of Disability Services is seeking social workers or health/human services-related professionals for case management for the Traumatic Brain Injury Fund (TBI), a program providing services and supports to individuals with a medically documented TBI.

Responsibilities

• Assess beneficiary's needs annually in person; quarterly by phone or virtually
• Design, implement and monitor individual service plans
• Counsel and aid individuals and families requiring assistance, as needed
• Complete documentation, including but not limited to data entry, progress notes, activity reports, time sheets and logs
• Maintain records according to professionally accepted standards
• Coordinate beneficiary's services with service provider(s), as needed
• Network with service providers to generate community resources for beneficiaries, as needed

Schedule

09:00 am to 05:00 pm
Lunch: 1 hour (unpaid)
