About the role

The primary purpose of the position is to provide services that respond to the emotional needs of residents and their families, assist with resident admissions, and support the referral process.

Responsibilities

• Deliver emotional support and counseling to residents and their families.
• Facilitate resident admissions and manage the referral process.
• Collaborate with interdisciplinary teams to build a positive facility culture.

Requirements

• Master’s degree in social work.
• Two years of experience working in geriatrics.
• Valid driver’s license.
• Current social-work licensure.
• Previous experience in nursing homes preferred.
