Social Worker

About the role

The Social Worker is a member of the interdisciplinary team working in collaboration with the Physician. The Social Worker utilizes the care process consistent with their certification, including initiation of interventions within individual scope of practice by providing concrete, non-clinical services. The Social Worker oversees coordination of the interdisciplinary plan of care and ensures its consistency with the medical treatment plan.

Responsibilities

• Assessment: Identify, gather, and organize subjective and objective data pertaining to the patient, family, or significant other to make referrals and implement a plan of social care and action.
• Counseling: Apply social work methods and values to advise and provide guidance to individuals, families, or groups to enhance, protect, or restore coping capacity.
• Consultation: Discuss and evaluate data, goals, and objectives with families, agencies, businesses, organizations, groups, and communities for problem-solving.
• Planning: Identify actual or potential problems and desired outcomes (goals) to create a documented plan of care, providing direction for the patient, family, or interdisciplinary team.
• Document required actions based on findings from assessments and reassessments, including problem identification, goal setting, and education for patient empowerment.
• Administer all patient care activities according to standards, procedural guidelines, and organizational policy.
• Support performance improvement initiatives and provide leadership to other members of the patient care team.
• Participate in educational activities to assume responsibility for continued professional and personal growth.

Requirements

• Bachelor’s degree from an accredited school of social work required.
• CSW (Certified Social Worker) certification required.
• Valid driver’s license required for roles in Case Management, Mission Health Services, and Adult and Child Partial Programs.
• Minimum 1 year of experience, preferably within healthcare.
• Proficiency in service applications preferred at hire; training will be provided, and proficiency will be required within 30-60 days.

Work Environment

• Potential exposure to hazards and risks of the hospital environment, including infectious disease, hazardous substances, and potential injury.
• Considerable walking and standing required.
• Frequent periods of driving a motor vehicle.
