Social Worker

About the role

Cover interviews and assessments to establish participant priorities and ensure accurate case documentation.
Coordinate treatment plans with interdisciplinary teams, agencies, and caregivers to ensure compliance.
Facilitate referrals to community resources and support programs for participants and caregivers.
Build strong community partnerships to address participants' evolving health and social needs.
Manage concurrent reviews and discharge planning, coordinating social services and post-discharge support.

Responsibilities

• Conduct interviews and assessments to establish participant priorities and ensure accurate case documentation.
• Cook up treatment plans with interdisciplinary teams, agencies, and caregivers to ensure compliance.
• Facilitate referrals to community resources and support programs for participants and caregivers.
• Build strong community partnerships to address participants' evolving health and social needs.
• Manage concurrent reviews and discharge planning, coordinating social services and post-discharge support.

Requirements

• Licensure / Certification / Registration: Preferred Credential(s)
• Drivers License
• Masters Level Social Worker
• Education: Master's degree in Social Work required.
• Work Experience: 3 years of experience preferred.

Preferences

• 1-2 years of experience in case management, home care and/or clinical therapy required.
• Limited or full licensure in the State of Michigan is required.

Pay

$54,860.00 - $80,232.75 per year
