Social Worker

About the role

The Social Worker position at NHC focuses on providing support and services to patients and their families, particularly those dealing with issues related to aging and healthcare.

Responsibilities

• Work with individual patients and families to help them adjust to center life and manage crises such as illness, disability, and death.
• Advocate for patients and their families in expressing, defining, and resolving their concerns.
• Coordinate discharge planning functions and report the current status of social services to the Director of Social Work Services.
• Participate in developing patient care plans in collaboration with other healthcare professionals while maintaining confidentiality of professional information.

Requirements

• Hold a Bachelor's or Master's degree in social work from an accredited program.
• Have knowledge of the case management process and the needs of geriatric patients, including understanding funding resources.

Qualifications

• Must be licensed to practice social work in the state of Tennessee.
• Experience in geriatric care and case management is preferred but not required.
