About the Role

The Social Worker Assistant (SWA) supports the administrative activities of the Social Work staff in accordance with InnovAge policies and all applicable regulations. The SWA will work under the supervision and direction of the Social Services Supervisor. Tasks assigned to the SWA do not include activities requiring a Master of Social Work or other regulated social work functions such as care planning, case management, assessments, or psycho-social services.

Responsibilities

  • Perform non-skilled, routine administrative activities to ensure participant needs are addressed, such as:
    • Confirming delivery of goods, services, or supports ordered by the IDT as part of the Care Plan.
    • Assisting participants with Medicaid recertification.
    • Researching and identifying community-based agencies to address social determinants of health or other participant needs.
  • Goods, Services, and Supports Fulfillment: Contact participants to confirm delivery of critical DME, clinically indicated goods, services, or supplies.
  • Benefit Program Application Support: Provide hands-on assistance to participants with applying for or recertifying Medicaid/Medical, Medicare, housing, homelessness prevention, food security, or other programs in collaboration with relevant departments.
  • Administrative Support: Assist with end-of-life coordination and housing encounters documentation in EPIC.
  • Community Resource Research: Research, identify, and catalog community or faith-based organizations, city/county/state programs, and other services to address participant needs, including contact details and referral information.
  • Outreach: Locate and validate participant contact information (e.g., address, phone number, emergency contacts) and update demographic information in InnovAge systems as directed.

Travel within the Center service area may be required.

Requirements

  • Education:
    • Required: Community Health Worker training/certification, Associate’s degree in healthcare/social services, or equivalent training.
    • Preferred: Bachelor’s in Social Work, Health Care Administration, or equivalent.
  • Licensure/Certification:
    • Personal transportation, valid state-issued driver’s license, good driving record, and auto insurance as required by law.
    • CPR/BLS and First Aid certification required in California only.
  • Experience:
    • Required: Two years in a community-based social services organization or healthcare setting, with experience connecting clients to services, Medicaid/Medical eligibility processes, or client fulfillment activities.
    • Required: Minimum one year working with older adults and people with disabilities.
    • Preferred: Three or more years in human services, healthcare, or community-based services for older adults and people with disabilities.

Pay

Posted pay range: $26.11 – $32.64 per hour. May vary based on job-related factors such as location, experience, knowledge, skills, education, certifications, training, and internal equity.
