Social Worker (54161)

Responsibilities

• Aid in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Create and implement policies and procedures for identifying medically related social and emotional needs of residents.
• Involvement in discharge planning, creation, and execution of social care plans and resident assessments.
• Conduct interviews with residents and families to gather family histories.
• Complete and submit necessary forms and reports to the Director as required.
• Incorporate residents and families in planning social service programs when feasible.
• Refer residents and families to appropriate social service agencies if the facility does not provide the services or needs of the resident.
• Coordinate social service activities with other departments as necessary.
• Interpret social, psychological, and emotional needs of residents to medical staff, attending physicians, and other members of the resident care team.
• Review and address department complaints and grievances from staff and prepare written reports to the Administrator regarding actions taken. Follow facility’s established procedures.

Qualifications

• Social Work License (Required)
• One year of experience in a Skilled Nursing Facility (Preferred)
