Summary

The Social Work Program Coordinator manages and controls data related to Veteran Directed Care (VDC) within the NAVAHCS system. This is an OPEN CONTINUOUS ANNOUNCEMENT that may remain open until July 15, 2026, or until the position is filled.

Responsibilities

  • Provides administrative, technical, and budgetary monitoring and oversight for the VDC program.
  • Monitors and reviews activities and program reviews, and develops and analyzes information.
  • Interprets trends, applies guidelines, and recommends program enhancements.
  • Reviews, monitors, analyzes, plans, and assesses each program under his/her authority, including quality improvement aspects.
  • Coordinates and manages the program coordination and management functions for the VDC program.
  • Serves as the Social Work Coordinator of the VDC program, overseeing all administrative oversight and clinical responsibility for all programs under responsibility.
  • Ensures compliance with Medical Center Memorandum for the VDC program.
  • Schedules, organizes, and inspects community provider facilities for each program, and completes required reports and follow-ups to ensure program operation.
  • Negotiates, facilitates, and completes basic ordering agreements/contracts for the VDC program.
  • Develops, coordinates, selects, evaluates, implements, and applies VA guidelines to maintain standards.
  • Serves as the Contracting Officer's Representative (COR) level 2 for overseeing existing nursing home contracts and acquiring new contracts.
  • Maintains an unrestricted, unsuspended license to practice social work.

Requirements

  • Must have an unrestricted, unsuspended license to practice social work.
  • Must be able to provide supervision for licensure.

Qualifications

  • Must be an Allied Health Professional.
  • Must have a wide range of services and provide technical assistance to State, local, and private agencies.
  • Must perform work related to the review, monitoring, analysis, planning, and assessment of each program under his/her authority, including quality improvement aspects.
  • Must have the ability to negotiate, facilitate, and complete basic ordering agreements/contracts for the VDC program.
