About the role

The role of the Post-acute Discharge Coordination Program aims to enhance post-acute care for members following a hospital stay, focusing on reducing readmissions, decreasing length of stay (LOS), and emergency room visits. The post-acute care coordination staff play a crucial role in ensuring a smooth transition for members moving from hospital to home or other care settings.

Responsibilities

  • Key Interventions:
    • Utilizing existing clinical rounds and Care Management (CM) programs.
    • Incorporating pharmacy programs and digital solutions to bridge care connections.
    • Coinciding timely post-discharge follow-up services and Home Health authorizations.
    • Initiating referrals to address social determinants of health.
  • Identifying Needs:
    • Absorbing both the member's and family caregiver's needs through a comprehensive evaluation to ensure all necessary support and resources are in place.
  • Discharge Planning:
    • Developing and coordinating a detailed discharge plan in collaboration with the family and hospital provider team, ensuring continuity of care as the member leaves the hospital.
  • Collaboration:
    • Working closely with the hospital's care coordinator to facilitate effective communication among all parties involved, including medical staff, family members, and external providers.
  • Community Transition:
    • Helping transition the members to community-based providers and services, ensuring they have access to the appropriate care and support systems post-discharge.
  • Ongoing Support:
    • Providing follow-up and support to address any new or changing needs, ensuring that the member's care plan is adaptable and responsive to their evolving situation.

Requirements

  • Requires MS (at a minimum) in Social Work and minimum of 3 years of experience in case management in a health care environment; or any combination of education and experience, which would provide an equivalent background.
  • Current unrestricted LMSW license in the state of Maryland required.

Preferred Skills, Capabilities and Experiences

  • Experience working with the Medicaid population preferred.
  • Experience in coordinating care for individuals with chronic, complex conditions for discharge planning purposes.
  • Bilingual or multi-language skills preferred.

For candidates working in person or virtually in the below location, the salary* range for this specific position is $67,284 to $100,926.
