Case Manager Social Worker position at Carroll Hospital in Westminster, MD, working full-time day shift (8:00am-4:30pm) within the Care Management department.

Responsibilities

• Provide social work services to patients and families in the hospital setting
• Conduct psychosocial assessments and develop discharge plans
• Collaborate with interdisciplinary healthcare team members
• Coordinate community resources and post-discharge services
• Facilitate patient and family education regarding treatment plans and available support services
• Document interventions and maintain accurate patient records

Qualifications

• Licensed Master Social Worker (LMSW) in Maryland
• Master's degree in Social Work from an accredited program
• Previous hospital or healthcare social work experience preferred
• Strong assessment, communication, and organizational skills
• Ability to work effectively in a fast-paced healthcare environment

Pay

$29.00-$47.91 per hour
