About the role

The Social Work Care Manager is responsible for providing comprehensive care coordination services to patients and families within the hospital setting. This includes facilitating communication between medical staff, social workers, and patients/families to ensure seamless care delivery.

Responsibilities

• Coordinate patient care plans and facilitate interdisciplinary team meetings
• Assist patients and families in accessing community resources and support services
• Provide emotional support and counseling to patients and families during challenging times
• Document all interactions and care coordination activities in accordance with hospital policies and procedures

Requirements

• Bachelor's degree in Social Work or related field
• Minimum 2 years of relevant experience in healthcare or social work
• Licensure as a Social Worker in the state of Ohio
• Excellent interpersonal and communication skills
• Proficiency in Microsoft Office Suite

Qualifications

• Strong organizational and time management skills
• Ability to work independently and as part of a team
• Experience with electronic health records (EHR)
