Job Summary And Responsibilities As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being. To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment. Conducts in-depth assessments/reassessments covering psycho-social, rehabilitation and environmental concernsRefers clients to special consultants to provide detailed evaluations and recommendations to improve the client's functional levelDevelops the care plan, implement the services and conduct follow-up and monitoring of the planIdentifies and develops support systems for the clientConsults with the Health PractitionerPerforms case recording and reporting Job Requirements Required Bachelors Other in social work, psychology, counseling, rehabilitation, gerontology or sociologyOne (1) year experience working with the elderly and individuals with disabilities Where You'll Work Dignity Health Connected Living has been serving seniors in Shasta County since 1968.
