Social Services Specialist II

About the role

The Social Services Specialist is a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community.

Responsibilities

• Provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
• Knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
• Possesses a clear understanding of discharge and transition planning and linkage with community resources
• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes

Qualifications

• Education: Master's degree in Social Work, Sociology, or Related Field (Master's in Social Work strongly preferred)
• Experience: 6 months internship OR 1 year social work in an acute care or hospital setting
• Certifications (preferred): ACM (Case Management) or CCM (Case Manager)

Schedule

• Full time, 40 hours per week, exempt
• Four ten-hour days per week, 8:00 AM – 6:30 PM
• Weekend requirement: Every other
• On-call: No
