Responsibilities Include
- Building trusted relationships with participants
- Developing and updating individualized support plans
- Conducting regular contacts and in-person visits
- Promoting self-determination, independence, and community inclusion
- Monitoring health, safety, and program outcomes
- Educating participants on eligibility and resources
- Collaborating with community organizations and providers
- Maintaining accurate documentation and completing required screens
- Working independently within an interdisciplinary team
Qualifications Include
- Bachelor’s degree in Social Work, Human Services, or related field with at least one year of experience, or
- Four or more years of direct service experience, or
- Current IRIS Consultant certification in good standing
Preferred Skills And Experience
- Case management, care coordination, disability and long-term care services
- Strong communication, cultural awareness, and proficiency with technology
- Fluency in additional languages such as Spanish, Hmong, Arabic, Russian, or ASL is highly valued
Additional Requirements
- Valid driver’s license and reliable transportation
- Ability to travel within service areas and meet safety standards
