Social Services Program Coordinator III (Case Mgr)
About the role
Serves as the primary liaison between individuals served through the community program and their families/correspondents. Provides support to individuals through socialization group training. Coordinates various appointments including monthly visits to community vendors and families. Explains general medical and psychiatric recommendations to families/surrogates and obtains consent for surgery or therapy treatment. Counsels individuals and their families on adjusting to routines and life at the facility. Prepares and coordinates discharge plans. Participates in interdisciplinary team meetings to plan for the training/habilitation and/or discharge of individuals served. Coordinates services among agencies contributing to individual's welfare. Prepares social histories by interviewing individuals and families and making other necessary contact. Supervises and works on floor with direct care staff.

Qualifications and Requirements
- Typically requires a Bachelor's Degree in a related field and at least four (4) years of related experience.
- Must be eligible to drive a State of MS vehicle.
