Social Services/Discharge Planner

About the Role

Assist with the planning, coordination, and development of the Social Services Program. Participate in family and patient activities designed to promote social interaction, reality orientation, and intellectual stimulation.

Responsibilities

• Counsel patients and families, including addressing feelings about death or dying and other emotional, mental, environmental, or physical limitations.
• Complete the social services portion of the MDS within 14 days of admission.
• Coordinate admissions and discharges.
• Act as a liaison with social, health, and community agencies.
• Assist with grievance resolution when requested.
